My only question would be: When the symptoms did not respond to the corticosteroid, did the patient return to the doctor to seek further treatment or just say "oh well, it didn't work" and continue on until the condition worsened to the point of serious problems.
Most doctors set a follow-up appointment after prescribing any type of medication for the sole purpose of determining if the course of treatment was effective or needs to be evaluated further. As usual, there is a lot of left out info in this case beyond a simple "oops! here's 3 million bucks"
