With the recent NASA airline pilot study story and the comments in this article, it's obvious there is a lot that needs to be done. Just maybe some of the noted improvements came about due to the study by NASA; it may have woken a few people up.

This article also indicates the FAA is proposing a 'penalty-free safety incident' reporting system. Well, if there IS a penalty for reporting safety incidents, that seems foolish, and no wonder the NASA study indicates that things are much worse than reported. Have a nice flight!
