As far as fixing the problem, the industry needs to go back to its roots. Start with realizing insurance companies buy risk, period. The next step is to spread the risk through the law of large numbers, remember that one from insurance 101?
Problem is, these two concepts do not fit into modern business models, so we decline as an industry.
