What most people don't understand is that the ride op has very little control over the ride once it actually starts. 95% is completely automated. Sometimes it's possible to hit "emergency stop" but often it won't do anything once the ride gets past a certain brake point.
The rides were always inspected by the top maintenance personnel every morning, and every ride op did a walk-through before the park opened. However, it's doubtful something like this would have been seen by anyone that wasn't a mechanic.
My heart goes out to the poor girl, and I pray that Six Flags do more than offer her "lifetime care."
