On February 05, 1999 (Friday) ESI subcontracted this case. During our investigation, we obtained 30 minutes of videotape during this day of surveillance. The claimant was noted running errands and going to a gym in Fountain Valley. As the investigation we were informed the District Attorney had accepted the Fraud case and would seek prosecution.
