A federal safety official says a combination of construction and design problems triggered the collapse of a Jacksonville, Fla. parking garage last December that killed one worker.

The federal Occupational Safety and Health Administration fined three contractors nearly $193,000 on June 3 and cited them with nine safety violations.

Jeff Romeo of OSHA says they failed to make sure the structure could handle the weight of concrete being poured on the top floor. He says they also removed reinforcement supports without approval, and that the building’s design and quality of construction were flawed.

Two of the companies plan to appeal.
