Beginning on March 25, 2004, the Arizona Department of Insurance opens its office in Flagstaff every Thursday for northern Arizona residents seeking licensing services and general insurance consumer assistance.
Services are available at the following times and location:

Thursdays, 10:00 AM to Noon and 12:30 to 3:00 PM
Individuals applying for an insurance producer (agent) license and individuals seeking to renew an existing insurance producer license can now conveniently take advantage of on-the-spot review of applications in the Flagstaff office during the posted hours on Thursdays. Fees must be paid by check, money order or by cash in the exact amount required.
Service in Flagstaff is available for individual applicants – business entities may continue to send license applications to the Phoenix Office for processing.
Letters of certification (a.k.a. letters of good standing) and other licensing services are available from our Phoenix Office.
