Either of those works, but also put it in the hazards (class code & rate) section with Addl Ins BF so if they see it on the rating portion it gets included.

He may have a deductible that is more than the damage to the customer's vehicle. Or... no coverage at all or an excluded driver. GKLL does require negligence on the part of the shop... which this would seem to be.

I would say that in many cases there is coverage through a crime policy or endorsement providing employee dishonesty on the victim's package policy or bop. I know in the claims I've seen, it was rarely publicized that there was insurance, and the repayment provisions were to reimburse the insurance ...
