I'm assuming that, since you're dealing with a large national construction company, there are insurance requirements built into the Agreement form. Ask for a copy of those insurance requirements because that's what dictates your client is obligated to provide. Anything beyond that - "NUTS"

Most third party cert management companies will have the COI come to them, vs. going directly to the actual Certificate Holder. FWIW, non-compete agreement in California usually don't hold a lot of water. Since your current and potentially future agency are all affiliates of a larger agency, there shouldn't be an issue in making a switch. However it should be the responsibility of the new potential agency to get this answer ...

The agency you currently work for owns that data. Copying, printing, whatever that data could be considered theft of intellectual property, which has severe legal implications. You should also have legal counsel review any current contract/agreement to see if there are any non-compete clauses.

There are a lot of variables as to whether a physical exam will be required. The amount of insurance, your age, answers to the required health questions, etc.
