I was talking to some agent friends this week, they each work at a different agency. We started discussing completed Acord forms. One agent said when they request additional insured broad form endorsement they place the request for broad form AI in the other coverages, restrictions and/or endorsements section on form 126. The other agent said they place the request in the Additional Interests section and for the name they write Broad Form.

It got me thinking and wondering where is the best place to write the request for broad form AI?

Either of those works, but also put it in the hazards (class code & rate) section with Addl Ins BF so if they see it on the rating portion it gets included.
