Find out what the extent of its coverage and the lenght of your elimination period. If every your elimination period is not yet done, you can call your insurance company and it will go through your records, and then they will give you instructions on how to file for claim. You need to submit all the needed documents such as the claim form, provider's notes, physician's notes and plan of care. If you own a reimbursement policy, your insurer will require you to submit proof of payment, bills, invoices and receipts.
