Start with addressing the payment methodoloy to emergency care providers. As it stands, they are paid 75% of what they bill. No cap on what you can bill though. This is leading to ERs submitting charges in excess of $15,000 for a single CT scan.
Same procedure on the outpatient level reimburses approx $600.00.
PPO networks could be established, but they need to be made attractive to the medical providers, instead of trying to only benefit insurers, and claimants need to be able to have these benefits as well. AFAIK, no insurance companies are offering these.
