How could the government not have known about this potential liability when they bailed out GM?

    According to the article…..

“An internal investigation into why it took GM more than a decade to identify problems with a defective ignition switch, led by lawyer Anton Valukas, blamed a lack of urgency in the engineering and legal departments but didn’t reveal any conspiracy to conceal facts.”

With a lack of urgency, why would they need to conceal facts? Isn’t the lack of urgency a reflection on the quality of leadership at GM?

Where is the responsibility then? Is it a Products/Completed Operations claim when a product fails and BI & PD are the result?
