People in severe pain are prescribed often opioids. They take them for a while to deal with the pain, and then their doctor abruptly discontinues the script. If they’re still in severe pain, to cope with the pain they may resort to buying street drugs, including heroin laced with fentanyl. I know respectable working class people who have resorted to buying street drugs. One is dead from an overdose; one is on her way there. System is broken.
