Horrible accident. Something tells me that the man was improperly trained on the equipment. It is just as bad as the Bumblebee Tuna guy that was caught inside an oven and baked with the tuna.
I have used these machines as recently as one year ago at a customer location. The doors have to be closed first, then the handle can operate the machine. It is virtually impossible for this to happen on the machine that I operated if there was only one operator. This could be an older machine (without those safeguards) which will (no doubt) lead to litigation because the owner of the machine did not update his equipment to a safer machine. The trash compactor was not as safe, but it was not owned by my customer.
It seems to me that if you operate a trash compactor at a recycling plant, you would make sure you had safe equipment before turning an employee loose on it.

Why update? It’s too expensive when there’s an inherent morale hazard at facilities like these. This is horrifying.
