It seems that many times employees know that they need to wear safety glasses, or protective clothing, or not remove safety features from machinery–but it happens because the employee may think that the protective equipment is too hot or too cumbersome.
When a risk is large enough and an effective insurance company loss control survey uncovers safety recommendations, it’s up to management to enforce those recommendations. Not sure how many losses occur because the employee forgot vs. the employee decided to disregard the provided safety equipment.
