Per early NTSB reports, this pipeline had thinned to only 30 percent of what it was originally. This is very likely internal or external corrosion. There are tools & techniques to prevent & track corrosion, but, some pipelines don’t take full advantage of them. A disturbing number of oil & gas pipeline failures are due to corrosion. The uneven techniques of pipeline integrity management among the different pipelines would be totally unacceptable in other fields, like commercial aviation. I’m surprised that some pipeline companies can still get insurance, with the blunders that have happened over the years.
Also, energy pipeline failures happen fairly often.
