A federal safety agency is proposing $42,000 in fines against a Waukesha, Wis., scrap yard where an employee was killed when he was struck by a forklift truck.
The Occupational Safety and Health Administration cited Waukesha Iron & Metal Inc. with several violations in the March 4 death of Kenneth LaChance.
LaChance was a 52-year-old maintenance manager at the scrap yard. OSHA said he died of head and neck injuries when he was struck by a forklift while hoisting an oxygen cylinder onto the machine.
The Journal Sentinel reports LaChance was working without head protection. OSHA says Waukesha Iron failed to ensure that workers were wearing protective helmets and that employees were trained in forklift operations.
The company declined to comment.
