A bench warrant has been issued for a Bowling Green, Ky., man who failed to appear for arraignment on fraud charges pursued by the Kentucky Office of Insurance.
Jaime Salazar, 31, of 1342 Bluelake Way, was indicted on one felony insurance fraud count in October and failed to show up for his Nov. 26 appearance in Warren Circuit Court.
According to court documents, Salazar applied for dental insurance with Health Resources Inc., in March 2006. He allegedly falsified the application, stating that he was married and had three children. After the policy went into effect on April 1, 2006, Salazar filed more than $2,000 in claims for purported dependents.
