The California Department of Insurance (CDI) Licensing Bureau unveiled a new Short Form Individual Application for insurance licenses. The form makes the application process faster and easier by allowing current licensees to add a Life, Fire & Casualty, Personal Lines or a Surplus agent license to their current license. The Short Form (441-17) is now available on the CDI website at www.insurance.ca.gov.
The Short Form consists of only two pages for an applicant to complete in order to add another license type to his or her current license. A life agent for example, could add a fire and casualty license type to his or her license just by filling out a two-page application as opposed to the six page Individual Application (Form 441-9). It is no longer necessary for the licensee to provide the background information the Individual Application requires each time. The Short Form two-page application is convenient for licensees and saves time and paperwork.
Information regarding this form or other license inquiries can be obtained at the CDI website www.insurance.ca.gov under the “most commonly asked questions” in the “License Questions and Answers” section.
If you need further assistance please contact CDI’s Producer Licensing Bureau at (800) 967-9331 or (916) 322-3555. The Call Center is available Monday through Friday between 7:00 a.m. to 5:00 p.m. You may also send an e-mail to License.Bureau@insurance.ca.gov. Please include your name, license number and contact information in all correspondence with CDI.
