Ohio Lieutenant Governor and Department of Insurance Director Mary Taylor announced that former insurance agent Alexis Prokup, of Perrysburg, Ohio, has been sentenced after pleading guilty to theft.
Department of Insurance investigators found that Prokup submitted more than 20 fictitious policy applications to American Family Life Assurance Company (AFLAC). She used the identities of family members, netting more than $12,000 in fraudulently obtained advanced commissions from AFLAC.
Prokup pled guilty to one count of theft on June 9, 2011, in Perrysburg Municipal Court. She was sentenced to180-days in jail (suspended), 18-months of community control, and ordered to pay restitution to AFLAC.
Prokup, previously surrendered her Ohio resident insurance agent license to the Department on July 7, 2010.
