Oregon’s Department of Consumer and Business Services announced that it has revoked the license of producer Derek J. Reynolds.
Reynolds was licensed in Oregon as a resident individual insurance producer from Sept. 5, 2000 to June 15, 2007. On June 15, Reynolds voluntarily surrendered his license. DCBS said Reynolds’ last recorded residence and business addresses were located in Lake Oswego, Ore. HE was subject to enforecement action for using a fraudulent, coercive or dishonest practice in his business.
In June and July 2002, as the manager of the Fidelity Brokerage Services LLC office in Tigard, Ore., Reynolds altered and falsified documents requiring signature guarantees; destroyed, altered and falsified annuity suitability forms, and backdated other documents, of the firm, or caused others under his supervision to engaged in such
conduct, in the course of preparing for an internal inspection and for the purpose of achieving an acceptable inspection result, as more particularly described in New York Stock Exchange LLC’s Hearing Panel Decision, number 05-59, dated Sept. 8, 2006, DCBS said.
