[ ] Proposed payment terms didn’t fit our budget.
[ ] General inattention to our firm’s needs.
[ ] Your style of presentation was offensive.
[ ] The content or format of the insurance proposal was difficult to comprehend.
[ ] The coverages, limits, or deductibles proposed were unsatisfactory.
[ ] The quality of your service after the sale was a concern.
[ ] The insurance company(s) that you used was not to my liking.
