2 Ill. Agents’ Licenses Revoked for Fraud

The Illinois Division of Insurance revoked the insurance producer license two agents recently for fraudulent activities.

Lynette J. Perkins of LaSalle, Ill., had been licensed to sell accident and health insurance since March 18, 2003. The licenses were revoked as a result of an investigation, which revealed that Perkins submitted two fraudulent applications to an insurer using a forged check, submitted an application on a nonexistent consumer, and failed to facilitate and aid in the examination.

Meanwhile, nonresident producer John Joseph Cox of Omaha, Neb., also had his licensed revoked. Cox had been licensed to sell life, accident, health and variable insurance since Feb. 5, 2003. The licenses were revoked as a result of an investigation which revealed that Cox improperly withheld $3,000 in insurance premiums, failed to facilitate and aid in the department’s examination, and failed to notify the director of a change in address within 30 days.
