A Shreveport, La., insurance agent has been arrested on charges of insurance fraud and forgery.
The Louisiana State Police Insurance Fraud/Auto Theft Unit arrested Stephanie Losey, 57, on one count of insurance fraud. Additionally, the Louisiana Department of Insurance fraud section investigators on Nov.3, 2011, served Losey with a notice of proposed action, license revocation and $5,500 civil fine notice.
It is alleged that Losey falsely and fraudulently completed and signed an application for a life insurance policy on an individual without his consent and without personally obtaining the information on the application. She is further alleged to have witnessed another party forge the name of the applicant on the policy.
According to department records, Losey was issued life, accident and health licenses in March 2001 and an industrial fire license in May 2001, all of which were summarily suspended on Nov. 3, 2011. Losey has 30 days from time of service to appeal this action.
